The absorption problem

Curcumin is rapidly metabolized and eliminated by the body, so plain turmeric or unenhanced curcumin powder taken alone has very low bioavailability, regardless of the dose on the label. Two approaches address this. Pairing curcumin with piperine, a black pepper extract often branded as BioPerine, can meaningfully increase absorption. Phytosome or liposomal delivery formulations, branded as Meriva or BCM-95 among others, bind curcumin to a carrier molecule for substantially better uptake. A curcumin product with neither enhancement is unlikely to do much regardless of the milligram count.

What the research actually supports

The most consistent evidence is for mild-to-moderate osteoarthritis joint pain, where several trials show benefits comparable to over-the-counter NSAIDs for some people, generally with fewer gastrointestinal side effects. General anti-inflammatory and antioxidant effects are well documented in lab and animal research and supported to a lesser degree in humans. Claims about major effects on cancer prevention or cognitive decline circulating online are based on much earlier-stage research and should not be the reason you take it.

Who should be cautious

Curcumin has mild blood-thinning properties, so anyone on blood-thinning medication or with upcoming surgery should talk to a doctor before starting it. It can also worsen symptoms in people with gallstones or bile duct obstruction.
